Application to erect a Memorial Headstone and/or surrounding Kerbs on a burial plot, or an engraved Plaque on an ashes plot (or to make any change to an existing facility), must be made in writing to the Parish Clerk.

The Form also asks for the Stonemason's details of the requested headstone or plaque to be attached - this can also be done via the Contact Us page.
